U nder this program, Tetra Tech provid ed urgent architecture and engin eering (A & E) recommendations, designs and construction supervision for the Caracol EKAM New Housing Settlement (Caracol EKAM) in the Northern Corridor. USAID’s acceptance of this final report coincides with and confirms their acceptance of all deliverables under this contract. All S ection F deliverables required under this contract have been uploaded to USAID’s DEC website. 2.0 BACKGROUND USAID , in cooperation with the Government of Haiti (GOH) , built the Caracol EKAM New Housing Settlement (EKAM) in the Northern Corridor to provide hous ing for households impacted by the 2010 earthquake and other critical groups in the Northern Corridor. This 750 - house settlement was constructed between October 2011 and October 2013. In March 2014 , USAID first became aware of deficiencies in both the hou sing and civil construction. A subsequent i ndependent a ssessment of a significant sample of houses revealed design and performance deficiencies including leaking roofs, missing or sub - specification roof fasteners, sub - specification foundations, substandard concrete reinforcement in wall construction, extensive material substitutions, poor quality doors and windows, minor cracking in walls, drainage issues, exposed water pipes, poorly designed and installed water tanks, improperly installed and substandard sewer pipes, and nonfunctioning solar security lights. I n October 2014 , USAID contracted Tetra Tech to provide independent Architectural and Engineering (A & E) and construction management services to inspect the EKAM site, provide A & E recommendations, manage and supervise remedial work, and provide support to US AID for contractual adjustments. 3.0 S COPE OF W ORK Tetra Tech ’s primary task under this contract was to provide urgent A & E services for the Caracol EKAM site. Tetra Tech’s secondary task was to assess and provide recommendations , as needed , for the civil work s USAID was providing at the Terrier Rouge and Ouanaminthe housing sites. These services were spelled out in the scope of work portion of our contract as follows: “ The objective of this contract is to provide urgent A&E construction management services to: (1) conduct an inspection of the entire Caracol EKAM project, including civil works and the 750 houses; (2) make A&E recommendations and oversee the completion of remediation of identified and reparable defects in work on the project; and (3) provide s upport to USAID for the development of related contractual adjustments under the relevant contracts. The new contractor will conduct the inspection of the project and then manage, supervise and monitor identified remedial work that will be undertaken by TH OR and CEEPCO at the EKAM site. It should be noted that since this settlement is already inhabited, so it is critical that any repairs pertaining to health and safety of the inhabitants be undertaken very quickly (fast tracked). Also, construction of repai rs will have to be closely monitored to prevent any harm to the inhabitants. Close coordination with all the parties involved in this work and with the local inhabitants will be a very important project consideration. AID - 521 - TO - 15 - 0001 Final Report Page 2 In addition to the tasks in Caracol EKAM, the new contractor will also undertake an assessment of Terrier Rouge and Ouanaminthe housing sites and provide recommendations for remedial action if required. In these two settlements, USAID is providing infrastr ucture (water & sanitation, roads, storm drainage and compacted plots) for the construction of 428 houses (Terrier Rouge: 242 houses; Ouanaminthe: 184 houses). The Government of Haiti in collaboration with the Inter - American Development Bank (IDB) and Food for the Poor (FFP) is constructing the houses .” 3.1 O BJECTIVES Tetra Tech’s initial objective at Caracol EKAM, as stated above, was to inspect the site, provide recommendations and oversee the remedial warranty repairs that were to be performed by the contrac tors responsible for the original work. THOR was the contractor for the housing and CEEPCO was the contractor for the civil works. As Tetra Tech’s in - country team worked through their field inspections at the Caracol EKAM site , it quickly became apparent that the effort required to repair the existing construction deficiencies being uncovered would be extensive. Additionally, it was noted that the current storm water drainage system, even with implementing the remedial repairs required to bring it to the original “as designed” condition, was undersized because the development of a banana plantation on the property to the southwest of the site had dramatic ally increased surface water flow to the site. The downstream drainage pat h was also undersized, which had the additional effect of backing up the stormwater into the site. C oncerns with the extent of the remedial repairs required at the Caracol EKAM site cause d USAID to revise Tetra Tech’s objectives. THOR and CEEPCO would not make the remedial warranty repairs. Tetra Tech would create design documents for select ed repairs that would be bid upon and the construction would be supervised by Tetra Tech. Tetra Tech’s scope of work at Terrier Rouge and Ouanaminthe was also revised a fter the original assessment , and it took over the monitoring of the remaining infrastructure work at both sites from PHS. That work is discussed in more detail below. 3.2 C A RACOL EKAM 3.2.1 INITIAL INSPECTION A ND FINDINGS Tetra Tech spent approximately one month at the end of 2014 assessing the field conditions at Caracol EKAM , reviewing design and other documentation , and comparing that information to the as - built conditions . This assessment identified multiple deficiencies that required replacement, repair or i mprovement across all aspect s of the work reviewed. Additional d etailed information on Tetra Tech’ s assessment can be found in document T - 20 EKAM Initial Field Assessment Report date d January 19, 2015. This report has been uploaded to USAID’s Development Experience Clearinghouse (DEC) for reference. 3.2.1.1 HOUSING AID - 521 - TO - 15 - 0001 Final Report Page 3 The housing issues reviewed and discussed in the above referenced rep ort related to roofing and framing of the roof s , doors/ windows and associated hardware , plumbing, slab/foundation and stucco. Short ly after the submission of the field assessment report , Tetra Tech was authorized to perform destructive testing on a random selection of houses to determine if the walls were properly reinfo rced per the original designs. That testing determined that the houses were lacking the reinforcing shown on the original design drawings. 3.2.1.2 SITE WORKS The site works reviewed were the water system, the sanitation/septic system and storm water drainage. The water system review looked at the well, well pump, pump building, water tower, underground water distribution system, service connections, house day tanks and how the system was being operated. The sanitation/septic system observations reviewed the sewer piping, septic tanks , leach fields (including soil conditions) and the operation of the system. The site drainage was reviewed to determine if the system was installed as originally designed and how current offsite conditions were affecting the ability of the drainage system to remove surface water from the site. 3.2.1.3 OTHER There was additional work that was reviewed in Tetra Tech’s field assessment report. This work did not neatly fit into either the site works or housing categories, so it was assigned to the “other” category. These portions of the work were the rainwater catchment system, street lighting and landscaping. 3.2.2 RECOMMENDATIONS, DES IGNS AND O VERSIGHT USAID reviewed the initial inspection and findings discussed above and determined that the repairs required were too ext ensive to be undertaken by the original contractors, THOR and CEEPCO. USAID directed Tetra Tech to put together design documents for the repair work selected. These documents would then be bid and , upon award of a contract, Tetra Tech would provide cons truction oversight of the selected contractor. 3.2.2.1 RECOMMENDATIONS After USAID reviewed Tetra Tech ’s initial findings , they selected the repairs and upgrades that they determined would best serve the community. The following repairs and upgrades were selected by USAID for Tetra Tech to provide design documentation for : • Reinforce the masonry walls of the houses to imp rove their structural integrity • Repair the roof ing and roof framing • Replace damaged doors and windows (shutters) and selected hardware • Rep aint the exterior of all houses • Replace the individual platform sanitary systems with a site - wide system • Make improvements to the water tower • Upgrade the water pump house and install a chlorine metering system • Replace all of the water distribution piping f rom the water tower to the houses • Repair/replace selected existing onsite drainage culverts and swales • Redirect water from the banana plantation around the site AID - 521 - TO - 15 - 0001 Final Report Page 4 Refer to Tetra Tech’s assessment report for more detailed information on the selected recommendations. 3.2.2.2 DESIGNS It was determined that the designs would be separated into discrete packages, one for the housing repairs, one for the sanitary repairs , and one for the other civil works which included the domestic water distribution system and the storm water management designs. 3.2.2.2.1 HOUSING The housing repairs design package called for several repairs to the roof. Those repairs included the roofing itself and the r oof framing. The metal roofing work included installing additional fasteners, patching leaks and installing/reinstalling flashing. The roof framing work included installing additional fasteners, additional wood blocking and properly securing the framing to the masonry walls. Door and window repairs were also included in the housing repair package. This work entailed replacing damaged door s and windows (shutters), refinishing the doors and windows, the installation of additional hinges on the exterior doo rs, and the installation of concrete thresholds at the exterior doors. Note that during construction the work to the doors and windows was revised to only include the refinishing of the exterior doors and windows. The final and most significant housing re pair work was to reinforce the masonry walls based on projected seismic activity. Tetra Tech initially came up with a design that installed steel strapping around the houses which would be covered by plaster and therefore not be visible. Because this wor k required extensive interior and exterior work on occupied houses, USAID requested we look at different options and brought Miyamoto to the site to provide their seismic design expertise. Miyamoto suggested that 4 shear walls could be used to provide the required reinforcement and could all be completed from the outside of the house. Tetra Tech subsequ ently subcontracted Miyamoto to provide the design of this suggested repair. Note that during construction unforeseen conditions required modifications to the design which resulted in additional construction costs and an extension of the construction duration. 3.2.2.2.2 SANITARY Tetra Tech initially suggested that the sanitary system collect effluent at each platform and that it be pumped through a force main to a le ach field. USAID felt that it would be problematic to have multiple pumps and asked that this design be revisited . The design that was bid allowed the effluent to flow via gravity to a central lifting station where it is pumped to the leach field. Due t o time constraints in looking at multiple options, survey requirements and other variables, USAID agreed that the design of the sanitary system should be bid as a design - build. This system replaced the existing thin walled sanitary piping from the houses to the existing septic tanks in each platform. The existing septic tanks in a platform were then tied together at a manhole with the effluent then flowing to the sanitary main in the street. The sanitary main lines were provided with manholes at a maxim um distance of 200’ to allow for maintenance. All of the effluent from the mains flows to the lifting station where two pumps alternate pumping the effluent t o the leach field for treatment. Discharge from the leach field is to the primary canal. During construction it was decided that the existing large septic tanks in the platforms be replaced with plastic tanks and that the small existing septic tanks in the platforms be bypassed. AID - 521 - TO - 15 - 0001 Final Report Page 5 During construction, the contractor, DFS, did not agree that this work was to be a design build. It was therefore agreed that DFS would provide survey information and Tetra Tech w ould take the design - build documents and turn them into a full construction set of documents. 3.2.2.2.3 OTHER CIVIL The other civil works primarily consisted of improvements to the water supply system and the installation of a canal around the site to help alleviate flooding. For the water supply system design, t he buried schedule 5 water supply piping was replaced with schedule 80 piping. The design originall y had the wa ter piping inside the platform s but during construction it was decided to move the water lines to the outside of the platforms to limit their crossing of the sanitary lines. Additionally, the water main from the water tower was rerouted to ali gn with proposed future development in that area. Shut off valves were placed at each platform so the day tanks at each house could be filled every other day, with ½ the residents receiving water each day. Water meters were also installed at each house t o monitor usage. The well pump house was repaired and a chlorine metering syst em was installed. The well head had a cap installed. The water tower had structural repairs designed to resolve defects and the platform at the top of the tower was enlarged to allow for access around the entire tank. The PVC riser was replaced with a galvanized riser. The site drainage system design routed the water from the banana plantation around the site in a canal that was designed for a 10 - year flood. The canal was designed with four culver t s that utilized RCP to take the water under existing roads around the community. Onsite swales were to be restored to the original design. Post bid, several changes were made to the site drainage design. The secondary canal at the back of the site was stabilized with a riprap wall to prevent erosion. The canal taking the storm water from the banana plantati on around the site was rerouted and the culverts were changed from RCP to box culverts. Note that th e onsite swale work was removed from the contractors’ scope at this time, but additional onsite swale repairs were added back into the project in 2018. Off - site improvements to the secondary and primary canals were also added to DFS’s scope of work post b id. I n 2018 road restoration work was added based upon requests from the community. For this work approximately 6” of the existing road surface was removed and replaced with gravel. 3.2.2.3 OVERSIGHT In January 2016 Tetra Tech submitted their Quality Assurance ( QA ) plan to USAID which described the methodology we would use during construction to monitor DFS’s work and progress. The following items are covered in this QA plan: • Meetings and reports • Scope verification • Codes and Standards • Field observations • Testing • As - built drawings • Environmental Compliance • Quality control documents AID - 521 - TO - 15 - 0001 Final Report Page 6 • Health and safety • Project schedule • Submittal review • Invoice review • Closeout procedures For additional information on any of the above oversight issues please refer to our QA plan which ha s been uploaded to the DEC site for reference. Below is a brief overview of some of the oversight details. USAID awarded the construction contract to DFS in September 2015 and in October DFS began mobilizing to the site and submitting administrative and technical submittals for the project. On November 20 th DFS was issued a partial Notice to Proceed (NTP) by USAID. This allowed them to start work at Terrier Rouge and the water tower at EKAM. Because work was authorized to begin , Tetra Tech held their f irst project meeting with DFS on November 26, 2015. All weekly construction meeting reports were transmitted to USAID via email on a weekly basis. These meeting reports detail the particular construction concerns being addressed at the time of their crea tion . That granular detail is excluded from this final report. Tetra Tech staff monitored construction activities associated with the partial NTP on a daily basis. Additionally, Tetra Tech began reviewing submittal, payment applications, requests for inf ormation (RFI) and other documents as needed. These daily activities and tasks are summarized in Tetra Tech’s weekly reports which were transmitted to USAID weekly via emai l . Part of the design requirements of this project was for the contractor to complete work on a model house prior to being allowed to start that portion of the housing work. As the shear wall installation work was on the critical path, DFS began shear wall w ork on three model houses which were selected because they covered all of the shear wall conditions. Miyamoto oversaw the fabrication of these shear wall “mock ups”. This process took several months to complete, but this step was necessary to ensure that DFS understood clearly what was expected as they would have to fabricate 3000 shear walls for this project. In anticipat ion of work starting on this project and , in particular , the housing work , Tetra Tech began interviewing local engineering firms to pro vide full - time on - site construction monitoring and community liaison services. Tetra Tech selected L’Atelier to be its in - country field inspectors . In February 2016 , L’Atelier started training to become familiar with the project scope and their responsib ilities. In March of 2016 , Tetra Tech’s field inspectors began surveying each house to document the actual existing conditions prior to DFS starting their work. These inspections and the documentation of the repairs completed were provided to USAID as a part of DFS’s closeout documentation for the Housing works. With its staff fully trained and work underway , Tetra Tech continued monitoring construction progress as DFS began additional portions of the work. The work was rolled out as follows: 1 Shear walls 2 Canal work starting with the secondary canal then culverts AID - 521 - TO - 15 - 0001 Final Report Page 7 3 Sanitary work starting with the manholes 4 Roof repairs 5 Water system repairs 6 Road repairs Throughout the project , Tetra Tech monitored construction and kept USAID informed of progress and concerns. If concerns were not being addressed by DFS, Tetra Tech issued a non - conformance report on the issue to highlight its importance, bring it to resolution and prevent it from becoming a punchlist item . When a portion of the work was complete our team inspe cted the work and provided DFS with a final punchlist that was finished before the work was considered complete. These non - conformance reports and all other construction documentation are included in DFS’s closeout documentation . 3.2.3 S UPPORT FOR CONTRACTUAL A DJUSTMENTS Because USAID decided to not have the existing contractors, CEEPCO and THOR, provide the remedial repairs , t his portion of Tetra Tech’s original scope of work was not required and our efforts were refocused on monitoring the construction repairs undertaken by DFS as discussed above in oversight. 3.3 TERRIER R OUGE AND OUANAMINTHE Tetra Tech was asked to review the civil works being undertaken by CEEPCO at Terrier Rouge and Ouanaminthe. Upon our completion of this review we were tasked with monitoring CEEPCO’s completion of the repairs to the water towers and septic tank installations at both sites and the remedial repairs to the leaching fields at Terrier Rouge. When this work was completed Tetra Tech worked with CEEPCO and USAID to closeout these pr ojects. The O&M manuals for this work were given to Tetra Tech by CEEPCO, but USAID never took possession of them. PDF’s of these manuals are submitted as an annex to this document for reference. Additionally, Tetra Tech was asked to provide designs at T errier Rouge for a community wide sanitary leach field system , similar to the one at EKAM , and additional storm water mitigation designs. Tetra Tech completed these designs and provided them to USAID (document T - 144) but the work was never implemented. Th e initial bid package for the repairs at EKAM also included work at Terrier Rouge and Ouanaminthe. This scope of work was not designed by Tetra Tech, but the construction monitoring of the work was included in our scope. The work at Terrier Rouge consisted of grading and stormwater improvements, pond dewatering, and main road swale improvements. The grading and Stormwater improvements were completed by DFS as described with some changes to the fence details. The pond dewateri ng scope was removed from DFS’s contract and not done. The main road swale work was revised by USAID to include only the main road swale across the street from the site and not the scope shown on the bid drawings. The work at Ouanaminthe, which consisted of access road improvements, was undertaken by a government of Haiti agency and was therefore removed from DFS’s scope of work. 4.0 ATTACHMENTS AID - 521 - TO - 15 - 0001 Final Report Page 8 1.
